NOTICE The Idaho Department of Health and Welfare, Division of Public Health, HIV, STD, and Hepatitis Section is seeking a maximum of two (2) qualified subgrantees to implement a Viral Hepatitis Community Health Patient Navigator program. The primary goal of this program will be to identify people at risk for hepatitis C (HCV), specifically those disproportionately impacted. The program will support a complete HCV medical evaluation and successful HCV treatment, including education to prevent reinfection. The Community Health Patient Navigator program may also support the overall disease burden of chlamydia, gonorrhea, syphilis, and HIV through innovative clinical practices and patient education. The funding parameters can be found in each subgrant application packet, in the Budget Proposal section. Funding for each subgrant will be based on satisfactory completion of the Scope of Work activities and budget justification.
