NOTICE OF INTENT TO SUBMIT STATE PLAN AMENDMENTS AND SOLICITATION OF PUBLIC INPUT The Idaho Department of Health and Welfare Division of Medicaid (Department) provides public notice of impending changes to the Idaho Medicaid State Plan, as required by 42 C.F.R. § 447.205. The Department assures these changes are in compliance with 42 CFR§ 440.345 and 42 CFR§ 447.204, and with the provisions of section 5006(e) of the ARRA of 2009. Medicaid has solicited input from Idaho's Tribal representatives regarding these changes in compliance with section 5006(e) of the American Recovery and Reinvestment Act of 2009 and its Tribal Consultation Policy. The Department also assures that individuals under twenty-one (21) years of age, pursuant to EPSDT, may receive additional services if determined medically necessary and prior authorized by the Department. The Department also assures that, in compliance with federal law, Tribal members residing in Idaho can voluntarily participate in the Healthy Connections Program, but do have the option to be exempt and may select the Primary Care Physician of their choice. Tribal clinics are exempt from the requirements to participate in HCVC for their capitation payments. PROPOSED STATE PLAN CHANGES Idaho Medicaid intends to submit applications to the Centers for Medicare and Medicaid Services (CMS) for State Plan Amendments (SPAs) to modify the reimbursement and structure for the Primary Care Case Management Program, known as Healthy Connections, and align it with the Healthy Connections Value Care Program. This will further encourage providers to transition to the Patient Centered Medical Home (PCMH) model of care and work together by partnering with a Healthy Connections Value Care Organization. The goal is to improve participant health outcomes and provide more cost-effective care. Contracted provider requirements will be outlined in their specific signed agreement with the Department. * Proposed program changes for Healthy Connections are identified within the 3.1-F state plan pages. The changes allow providers to choose from one of three reimbursement levels, based on the PCMH activities they choose to perform. * Proposed reimbursement changes for Healthy Connections are identified within the 4.19-B state plan pages. The changes collapse the existing provider reimbursement rate into a flat monthly per member per month (PMPM) rate based on reimbursement level, and define quality performance improvement activities. * Proposed reimbursement changes to Healthy Connections Value Care are also identified within the 4.19-B state plan pages. The changes update definitions, modify the total cost of care methodology to allow for a statewide target rather than an individual target, and simplify the Quality Performance Program. The effective date for these changes is July 1, 2021. ESTIMATE OF EXPECTED CHANGE IN ANNUAL AGGREGATE EXPENDITURES Because the Healthy Connections program changes are simplifying the reimbursement rates into one PMPM rate per PCMH tier, there is no expected increase or decrease in annual aggregate expenditures. The proposed changes to the HCVC program utilize a shared savings approach, therefore, there is no expected increase or decrease in annual aggregate expenditures.
