NOTICE OF PUBLIC HEARING ON FEES NORTH KOOTENAI WATER DISTRICT The Board of Directors ("Board") of North Kootenai Water District ("District") will hold a public hearing on February 4, 2016 at 12:30 p.m. at the District office, located at North Kootenai Water District 13649 N. Meyer Road Rathdrum, ID In accordance with Idaho Code \u00DF 63-1311A, the purpose of the public hearing is to take public comments regarding a proposed new fee and/or increases, in excess of five percent (5%) over the amount last collected, for the following fees and/or charges of the District: (a) Capitalization fee for meter sizing larger than one (1) inch; and (b) fee charged for Qualified Licensed Professional Engineer (QLPE) review of plans and specifications pursuant to IDAPA 58.01.08. The increase to certain capitalization fees is the result of District review of the costs of the capital infrastructure of the water system ("System") of the District, including the acquisition of the System, costs in improvements to the System, changes in technology and the industry since acquisition of the System, capital costs expended and the necessary capital plan of the District. The increase in and/or establishment of the fee charged for QLPE review is to allow the District to recoup its actual costs associated with this service. These proposed fees and/or increases will apply to the entire System, including the area commonly referred to as Hayden Haven/Gem Shores. These proposed fees and/or increased fees will not apply to property in the District currently connected to the System. The Board will consider all written and oral communication in making a final determination of the appropriate rates and charges. Kootenai Hospital District d/b/a: Kootenai Health 2003 Kootenai Health Way Coeur d'Alene, ID 83814 Phase 2, Facility Master Plan Surgery & Emergency Department Projects Request For Prequalification Building and Specialty Contractors Pursuant to Idaho Code, Title 67, Chapter 28 (67-2805, paragraph 3(b)), Kootenai Health has determined that due to the complex nature of the Phase 2, Facility Master Plan Surgery & Emergency Department Projects (Projects) Idaho licensed building and specialty contractors shall meet preliminary supplemental qualifications if they intend to competitively bid any work that is related to the Projects as listed below: A. Steel studs/drywall work B. Heating, Ventilation, Air Conditioning (HVAC), and Controls C. Plumbing D. Electrical, Communications and Electronic Safety/Security Prequalification Requirements: Each interested subcontractor will be required to submit to Bouten Construction Company, Construction Manager/General Contractor (CM/GC), a completed copy of the Request for Prequalification (RFP) no later than Friday, January 29, 2016 at 12:00 PM (PST). The RFP responses will be evaluated in accordance with the established criteria as set forth in the RFP to determine which subcontractors will be eligible to submit bids to Bouten Construction Company for the applicable Projects. Only those firms meeting the minimum RFP Criteria and scoring a minimum 90 points will be eligible to submit bids. Optional requirement: At Bouten Construction Company's sole discretion, a current copy of the subcontractor's most recent audited financial statement may be requested and shall be submitted upon request. Following the prequalification process, a list of prequalified subcontractors will be published and only those firms who have been prequalified will be allowed to bid the Projects. Project Description: This prequalification process will apply to both projects as described below: Emergency Department (ED): Project scope will involve a 6,600 square foot expansion and a 15,000 square foot remodel of the existing emergency department along with extensive site and parking lot modifications. The project will include three phases of work starting with the expansion in June 2016, and ending with the third phase of remodeling in 2018. The main emphasis of this project will involve creating additional patient exam rooms to accommodate increasing patient volumes at Kootenai Health. Surgery Department: Project scope will involve a 1,500 square foot expansion and a 36,000 square foot remodel of the existing surgery department. The project will include three phases of work starting with phase one in May 2016, and ending with the third phase in 2018. The main emphasis of this project will involve creating three new operating rooms, rebuilding six existing operating rooms and creating new pre and post-operative patient space on the second floor of the medical center. The phasing and sequencing of construction will require careful coordination to ensure that patient, public, and staff safety is maintained at all times. Subcontractors will be expected to participate in and abide by all requirements related to safety, infection control, coordination of work, and Lean practices that will be implemented on the Projects. NOTICE OF PUBLIC HEARING NOTICE IS HEREBY GIVEN that the Kootenai County Board of Commissioners will conduct a public hearing at or after the hour of 6:00 p.m. on February 22, 2016, in Room 1A/B of the Kootenai County Administration Building, located at 451 Government Way, Coeur d'Alene, Idaho, to consider the following: Case No. ORA15-0001, a request to establish a new land use classification in accordance with Title 9 Zoning Regulations, Section 9-21-7 of the County Code. Specifically, the request would provide for a Resort Lodge, Conference Center, or Guest Ranch, and would be defined as follows. A facility, including a lodge and/or resort cabins, and other supporting ancillary buildings, with or without food service, that serves as a destination point for patrons and that relies on its rural location and the natural environment to provide recreational opportunities and activities such as horseback riding, hiking, fishing, and boating. If customarily incidental to the use, these facilities may also be used for meeting, recreation, education, and wedding and reception facilities. The use is proposed to be allowed by-right in the Commercial zoning district, and by Conditional Use Permit in the Agricultural, Agricultural Suburban, Rural, and Restricted Residential zoning districts. The Planning Commission held a public hearing on October 15, 2015 and deliberated on October 22, 2015, and voted to forward the request to the Board of County Commissioners with no recommendation. The Board of Commissioners held a public hearing on December 10, 2015 and unanimously voted to continue deliberations to January 14, 2016 at 6:00 p.m. The Board of Commissioners held deliberations on January 14, 2016 and unanimously voted to re-open the public hearing to a date in February.
