Heritage Health receives award for Kroc Rx program

Heritage Health’s Kroc Rx project earned the healthcare provider Qualis Health’s Award of Excellence: Outpatient setting.

The Heritage award is one of four given statewide by Qualis, a population health management organization.

“Our changing healthcare landscape requires increasingly vigorous efforts to improve healthcare delivery and health outcomes in Idaho and across the nation. The Qualis Health Idaho Quality Awards celebrate organizations that have transformed healthcare delivery for their communities — making a real difference for their patients and their families,” said Jonathan Sugarman, M.D., MPH, President and CEO of Qualis Health. “Their achievements exemplify progress towards critical aims for healthcare: better quality of care at a lower cost.”

The Kroc Rx project, developed by Heritage Health and offered in partnership with the Salvation Army Kroc Center, identifies and motivates patients at risk of developing diabetes. The patients attend office visits, exercise and coaching sessions at the Kroc Center, all under the guidance of Heritage Health professionals.

Other award-winners were St. Joseph’s Regional Medical Center in Lewiston, St. Luke’s Rehab in Boise and Bonner General Health in Sandpoint for their Stroke Alert project.
