Prequalification Requirements: Each interested Contractor will be required to submit to Bouten Construction Company, Construction Manager Advisor (CMa) a copy of the Request for Prequalification (RFP) no later than Wednesday, April 15, 2015 @2:00 p.m. (PDT). The RFP responses will be evaluated in accordance with the criteria below as needed to determine which Contractors will be eligible to submit bids to Kootenai Health. Project Description: The Project involves construction of one monument sign at the new north entry to the East Expansion and up to 4 other monument sings in various locations around the campus. The north entry sign will be approximately 36 feet tall and include the use of existing structural footings and masonry pier to support the sign. The sign will be constructed out of perforated metal and plexiglass, to create a monument with visible graphics day and night. The sign material, lighting, and any internal and supporting structure are all included in this scope of work. The design will be conceptually completed by the project architect, NAC Architecture. The Contractors will be responsible to complete the design without altering the final look of NAC's design, submit design for approval, and then execute all work associated with the monument. Schedule for the monument is very critical and contractors must adhere to the schedule dates on awarded contract for this work. The other potential signs include the primary interior sign inside the lobby with construction / aesthetics similar to the north entry sign described above. Site signage may include a monument at the corner of Ironwood and Highway 95 and other key locations on campus. Monument signs that are not attached to the building may also include structural footings and masonry piers. The phasing and sequencing of construction will require careful coordination to ensure that patient, public and staff safety is maintained at all times. Contractors will be expected to participate in and abide by all requirements related to safety, infection control, coordination of work and lean practices that will be implemented on the Project.
