Kootenai Hospital District 2003 Kootenai Health Way, Coeur d'Alene, Idaho 83814 REQUEST FOR PREQUALIFICATION OF PRIME CONTRACTORS, FOR KOOTENAI HEALTH PROJECTS Kootenai Hospital District in Coeur d'Alene, Idaho is inviting all interested Prime Contractors to respond to this request for prequalification (RFQ) for Kootenai Health projects proposed for calendar year 2013. The deadline for submittal of prequalification questionnaire is Friday February 15thth, 2013 at 12:00 p.m. local time. The scope consists of renovation projects located within the existing Coeur d'Alene campus and the campus in Post Falls. Both locations consist of multi and single story buildings. The intent of the RFQ is to prequalify contractors for work that requires specific experience and knowledge to safeguard patient health and operations. The projects will be competitively bid in accordance with Idaho Code for public works construction (as required by Idaho code Title 54, Chapter 19). Specific bid package descriptions and responsibilities will be an integral part of the bid documents. The intent is to award separate contracts for the work generally described below to the lowest qualified responsive bidder. In addition to insurance, bonding ability and adequate licensure, the following standards will be used to evaluate the prospective bidders: \u2022Satisfactory experience of no less than three (3) hospital construction projects where company has performed any work within the last five (5) years. Include current contact information for the owner and architect. Describe in detail the scope of work contracted, start/finish dates and construction related costs excluding associated equipment costs. \u2022Satisfactory experience of proposed construction superintendent listing no less than three (3) hospital construction projects this individual has managed in the last five (5) years. Include a detailed description of their management responsibility, the scope of work contract start and finish dates, construction related costs excluding associated equipment costs. NOTICE OF ADVERTISEMENT FOR BIDS Sealed bids will be received by the City Council of the City of Coeur d'Alene in the office of the City Clerk on or before Tuesday, February 26, 2013, until the hour of 4:00 p.m. of said day for the 2013 MCEUEN CONSTRUCTION PROJECT Bids will be opened on said date at 4:00 p.m. by the City Clerk in the Old City Council Chambers at City Hall, 710 E. A Non-Mandatory Pre-Bid meeting will be held on Wednesday, February 13, 2013 at 10:00 a.m. in the Community Room at the Coeur d'Alene Public Library, 702 E. Front Avenue, Coeur d'Alene, Idaho. All prospective bidders are strongly encouraged to attend the pre-bid meeting. The meeting will present a detailed explanation of the project. All bids shall be presented or otherwise delivered to the City Clerk under seal with a concise statement marked on the outside thereof identifying the expenditure to which the bid pertains. All bids shall contain one of the following forms of bidder's security in an amount equal to 5% of the amount of the bid: (a) Cash, (b) Cashier's Check made payable to the City, (c) Certified Check made payable to the City, or (d) Bidder's Bond executed by a qualified surety company, made payable to the City. The City, in accordance with Title VI of the Civil Rights Act of 1964, 78 stat. 252, 42 U.S.C. 2000d-4 and Tittle 49, Code of Federal Regulations, Department of Transportation, Subtitle A, Office of the Secretary Part 21, Nondiscrimination in Federally-assisted programs of the Department of Transportation issued pursuant to such Act, hereby notifies all bidders that it will affirmatively ensure that in any contract entered into pursuant to the advertisement, disadvantaged business enterprises as defined at 49 CFR Part 23 will be afforded full opportunity to submit bids in response to this invitation and will not be discriminated against on the grounds of race, color, national origin, or sex in consideration for an award.
