Social Security Checks, etc, as her birth certificate, driver’s license and Social Security cards do not match. (She currently receives her Social Security payments through debit card since she cannot open a bank account where the money could be deposited) She would like to formally and legally change her name to “Megan Lee Crosiar” for the above reasons.
