The Idaho Department of Health and Welfare's Suicide Prevention Program is offering funding for a new applicant to administer youth suicide prevention services through the Idaho Lives Project in specific middle/junior high and high schools statewide and already determined by the Department. The funding level being offered is $135,000 maximum for six (6) months. The opportunity is funded by Idaho state general funds. The APPLICATION DEADLINE is Monday,September 12, 2016. Applicants must provide the Data Universal Numbering System (DUNS) number with application or a statement affirming the Subgrantee will provide their DUNS number prior to subgrant award. Applications will be evaluated based on how the prescibed activities fit the applicant organization's mission, the applicant's family and/or experience with Idaho suicide prevention programs and resources and the applicant's ability to fulfill the required activities within the allotted award amount.
