NOTICE OF APPLICATION FOR TRANSFER OF FIDUCIARY CAPACITIES FROM COLUMBIA STATE BANK TO AN AFFILIATED TRUST COMPANY Columbia State Bank ("Columbia"), a Washington state-chartered bank, and West Coast Trust Company, Inc.
