Kootenai Hospital District 2003 Kootenai Health Way, Coeur d'Alene, Idaho 83814 REQUEST FOR PREQUALIFICATION OF PRIME CONTRACTORS, FOR KOOTENAI SERVICES BUILDING 1ST FLOOR RENOVATION Kootenai Hospital District in Coeur d'Alene, Idaho is inviting all interested Prime Contractors to respond to this request for prequalification (RFQ) for a Kootenai Services Building project proposed for mid-summer 2014. The deadline for submittal of prequalification questionnaire is Friday May 23rd, 2014 at 12:00 PM local time. The scope consists of a single renovation project affecting 4000 square feet of an occupied building. The project site is located within the Kootenai Services Building at 2205 Kootenai Health Way within the existing Coeur d'Alene campus. The Kootenai Services Building conducts normal business operations during the daytime hours of 8 to 4:30, interested Prime Contractors must be able to conduct the renovation project outside of the afore mentioned times. The intent of the RFQ is to prequalify contractors for work that requires specific experience and knowledge to safeguard patient health and operations. The projects will be competitively bid in accordance with Idaho Code for public works construction (as required by Idaho code Title 54, Chapter 19). Specific bid package descriptions and responsibilities will be an integral part of the bid documents. The intent is to award a contract for the work generally described below to the lowest qualified responsive bidder. In addition to insurance, bonding ability and adequate licensure, the following standards will be used to evaluate the prospective bidders: o Satisfactory experience of no less than three (3) medical office projects where company has performed any work within the last five (5) years. Include current contact information for the owner and architect. Describe in detail the scope of work contracted, start/finish dates and construction related costs excluding associated equipment costs. o Satisfactory experience of proposed construction superintendent listing no less than three (3) medical office construction projects this individual has managed in the last five (5) years. Include a detailed description of their management responsibility, the scope of work contract start and finish dates, construction related costs excluding associated equipment costs.
