Active life transitions specialty therapy unit launched
COEUR d'ALENE - Ivy Court announced today the launch of an innovative therapy unit, Whispering Pines at Ivy Court, with specialized therapy staff that instructs and guides patients in achieving their individual rehabilitation goals while providing concierge services and all the comforts of home.
With a focus on returning patients home safely with strength and confidence, Ivy Court's Active Life Transitions Unit (ALT) is designed to meet the needs of patients that require short-term rehabilitation services. Implementing a holistic plan of care, ALT provides a personalized rehabilitation plan visualizing progress and benchmarks achieved during a patient's stay. Medical care is managed by multiple partnerships with local physicians and state-of-the-art therapy equipment provides ALT patients enhanced balance, strength training and functionality. Three local physician specialists provide unit oversight by serving on the ALT Physician Advisory Panel. Ivy Court's medical director, Dr. Susan Melchiore, is North Idaho's only geriatric specialist.
Setting this program apart from other rehabilitation programs is an emphasis on providing personalized care with all the comforts of home. At Ivy Court, a Guest Services Coordinator offers ALT patients concierge services to ensure their stay is comfortable and pleasurable and a culinary experience that caters to the patient's lifestyle, dietary needs and taste preferences. Specially trained nurses and therapists also present individualized patient training and instruction on maintaining health and independence after discharge to home, along with guidance for successful management of any ongoing medical needs and/or continuing the rehabilitation and strengthening process.
After discharge, ALT's Extend to Home Program follows up regularly with patients to see if any additional assistance is needed or if the patient is transitioning well and retaining functionality. To ensure continued success at home, staff also educates patients on available community resources and creates a home wellness plan that provides comfort to patients and instills confidence that health and well-being will be maintained.
