River City Hospice provides services for patients in their home who are facing a life limiting illness. Although the primary focus of River City Hospice is pain and symptom management, the individual needs and preferences of the patient and their family is always our top priority when a care plan is being developed. • Registered Nurse available 24 hours per day, seven days per week and other hospice team members as warranted.

• Other team members include: Medical director, spiritual counselor, social worker, volunteer, certified nurse aide and administrative support.

• Medications prescribed by the physician that are directly related to the hospice diagnosis.

• Medical equipment that is related to the hospice diagnosis and support the patient in their daily functions.

• Medical supplies that are needed to address symptoms or provide palliative treatment.

• Education for the family in the proper care of the patient and the stages of the disease process.

The nurse works closely with the physician and hospice medical director to ensure that the patient receives appropriate care and treatment as ordered.
